The MGH Revere HealthCare Center received a $50,000 grant from the Tufts Health Plan Foundation in support of its innovative Senior Wellness Program. Primary care providers at MGH Revere refer high-risk, socially isolated seniors with at least one chronic disease condition to a multidisciplinary team consisting of a registered nurse, a social worker and a health coordinator. The team works to identify risk factors and establish health behavior change goals focused on physical activity, social activation and self-management of chronic disease. Seniors also participate in tailored health coaching, psychoeducational groups, field trips and physical activities that include tai chi, gentle yoga and strength-training classes.
