PSC Forms
Please feel free to download a copy of the PSC by clicking on the link below or you can email us if you have any trouble doing so. We ask that you refer to our Scoring Instructions section before using the PSC in your home, school, or health-care setting. If you do research with the PSC, please send us a copy of any reports or papers you write. Our goal is to keep abreast of all the research with the PSC.
