The Ambulatory Practice of the Future (APF), a team-based, patient-centered medical home, provides an innovation laboratory within which clinical practice solutions for improved primary care are developed and tested.
Embedded within a state-of-the-art primary care practice affiliated with Massachusetts General Hospital, the Ambulatory Practice of the Future Innovation Learning Program (APF-ILP) is uniquely poised to serve as a clinical testing ground with the goal of identifying solutions to the most difficult challenges facing primary care today.
We do so by partnering with industry leaders to find ways to successfully address challenges in the areas of information systems, practice workflow, medical device utility, creation and integration, physical space design, payment reform and beyond.
Our initiatives are designed to be strategically aligned with institutional and national efforts and seek to explore the boundaries of practice improvement solutions in primary care.
The Innovation Learning Program (ILP) is part of the APF’s transformation underway within primary care deploying virtual care technologies and coordinated patient-centric, team-based care. ILP’s remit is to increase the workflow benefits gained from research and clinical collaboration with private sector device and technology companies in real-world patient trials run via the APF; then aggregate and re-share the resulting best practices with the participating practices through our learning management system.
Participating practices do not each have to reinvent an entire process or search for the right tools each time themselves. In addition to creating real cost- and labor-saving practice workflow and operational benefits, while maintaining the best patient experience, safety and quality outcomes, this approach allows each practice to focus on their unique cultural, disease, demographic and pharmaceutical requirements. They need not waste valuable time and resources in duplicate efforts. ILP manages the foundational underpinning and gives practices access to a live clinical practice in which to test their research.
