Massachusetts General Hospital has made arrangements for patients to receive the Shingles Vaccine. There are two sites where the vaccine will be given, Wang Ambulatory Care Center 6 and the Revere Health Center. The vaccine is given at a scheduled appointment time arranged through your primary care office.
Bulfinch Medical Group has prepared a questionnaire with general information about the vaccine and also some questions that will need to be answered to assure the vaccine is safe for you.
The questionnaire is attached below. Please complete and return to the office for your PCP’s review and approval. The office will then contact you to schedule an appointment.
