For more than 30 years, Massachusetts General Hospital Community Health Associates (CHA) supported the work of MGH Health Care Centers serving Charlestown, Chelsea, Everett, Revere, and the North End (affiliate) in delivering comprehensive, high quality, primary and preventive health services including wellness and health promotion to the low income and underserved populations living and working in these communities.
The Health Education and Promotion Programs engage MGH HealthCare Centers, community-based agencies, schools, individual volunteers, student interns, and patients in the design, implementation, promotion, and assessment of health education activities and services.
The Prevention Programs serve patients directly from an MGH HealthCare Center and its satellite locations. These programs feature case management, care coordination, patient navigation, community outreach, education and counseling, as well as patient confidentiality, all for the purpose of bringing effective prevention services to patients.
The Disease Management Programs are operated onsite at MGH HealthCare Centers. Led by a physician or a Nurse Case Manager, these programs strive to improve patient care outcomes and provide the much needed outreach services for the patients to manage a chronic disease well and live a healthy life.
The Wellness Center's goal is to enhance the primary care patients receive from MGH HealthCare Centers and help patients improve their well-being and quality of life through providing them with culturally sensitive, easily accessible, and affordable wellness programs and services.
