TopCare Patient Navigator Program

The TopCare program (Technology for Optimizing Population Care in a Resource-limited Environment) was established by MGH's General Medicine Department to increase cancer screening rates among vulnerable patients cared for in any primary care practice at MGH.

A key part of the TopCare Program is the navigation of patients who are at high risk of not completing preventive breast, cervical and colorectal cancer screening.

2014 Outcomes

TopCare served 996 patients.

155 cancer screenings were completed (77 mammograms, 47 colonoscopies and 31 Pap smears).

  • 109 procedure instructions

  • 5 accompanied to appointment
  • 4 financial assistance referrals
  • 1 childcare assistance
