To support the health centers, CHA develops innovative and evidence-based disease management, health education and promotion and prevention programs, as well as programs to improve access to care and primary care practice redesign. The programs and services are located at CHA offices at MGH Revere, at the health centers or in the community.
The Health Education and Promotion Programs engage MGH HealthCare Centers, community-based agencies, schools, individual volunteers, student interns, and patients in the design, implementation, promotion, and assessment of health education activities and services. The programs are ARCH, Celebrating Women, Living a Vibrant Healthy Life!, Smart Choices and Stay In Shape.
The Prevention Programs serve patients directly from an MGH HealthCare Center and its satellite locations. These programs feature case management, care coordination, patient navigation, community outreach, education and counseling, as well as patient confidentiality, all for the purpose of bringing effective prevention services to patients. Our prevention programs are the Family Planning Program and the School-Based Health Centers at Revere and Chelsea High Schools, operated by MGH Revere and MGH Chelsea, respectively.
The Disease Management Programs are operated onsite at MGH HealthCare Centers. Led by a physician or a Nurse Case Manager, these programs strive to improve patient care outcomes and provide the much needed outreach services for the patients to manage a chronic disease well and live a healthy life. Our Disease Management Programs are the Hepatitis C, HIV/AIDS and Suboxone Programs.
