Charlestown Family Support Circle

The Charlestown Family Support Circle was designed by members of the Charlestown neighborhood to provide clinical case management and care coordination services to the community.

Overview

The Family Support Circle is an outcome of Charlestown’s 2012 Community Health Needs Assessment. The mission is to provide supportive services to Charlestown youth and families who are at risk or in need of support, to ensure all Charlestown youth are able to develop and grow to reach their full potential.

About this Program

The Charlestown Family Support Circle was designed by members of the Charlestown neighborhood to provide clinical case management and care coordination services to the community.  The FSC Clinician is the central referral point for Charlestown families and residents, who have children and adolescents between the ages of 7-18 years old. The FSC Clinician works with families to determine their strengths, needs, and goals, as well as to provide families with referrals to appropriate services and treatment. Additionally, the   FSC Clinician remains in contact with families until they are connected to community resources while also providing a consistent, ongoing, community support. The program also works with Charlestown providers to improve care coordination and the way in which providers work together.

2015 Outcomes

  • CFSC provided 38 families with supportive case management services.
  • The CFSC clinician had 1,000 contacts with families, including home visits, accompaniment to court and school meetings, and coordinating referrals and services.
  • Twelve agencies sat on the task force to improve care coordination in the community.
  • CFSC received referrals from 13 organizations, including CharlesNewtown Housing Development, Mishawum Park Peabody Properties, Smart from the Start program, Harvard Kennedy Elementary School, MGH Charlestown Health Center, John F Kennedy Center, and the Charlestown Boys and Girls Club.
  • CFSC updated the Family Resource Guide.
