For more than 30 years, MGH Community Health Associates has worked with the MGH HealthCare Centers in serving Charlestown, Chelsea, Everett, Revere, and the North End (affiliate). CHA supports the work of MGH health care centers who care for patients through their life span. CHA delivers comprehensive, high quality, primary and preventive health services including wellness and health promotion to the low income and underserved populations living and working in these communities. In addition, CHA provides program development and support, clinical supervision, grant writing, contract and fiscal management, practice redesign facilitation, health education, and advocacy.
To support the health centers, CHA develops innovative and evidence-based disease management, health education and promotion and prevention programs, as well as programs to improve access to care and primary care practice redesign. The programs and services are located at CHA offices at MGH Revere site, at the health centers or in the community.
