NHP/MGH HealthCare Center Access Project (NHP
Access Project)
The NHP Access Project promotes access to primary care for residents of Charlestown, Chelsea, and Revere. Outreach workers, called Practice Liaisons
(PL), are located in primary care practices at the health centers to facilitate
and coordinate linkages among patients, providers, and health resources. NHP
Access is tailored to the needs of each health center. The Practice Liaison
is specialized in compiling and maintaining a list of community resources,
establishing relationships with community agencies, accompanying patients
to specialty medical visits or community agencies, and following-up on patient
"no shows."
