Suboxone services at MGH Chelsea and MGH Revere are provided by a team of primary care physicians who are certified to prescribe suboxone (buprenorphine) and a nurse case manager who closely monitors and supports patients in addressing their opiate addiction.  The nurse case manager creates a much-needed link between the health centers and community-based services necessary to identify and treat these individuals, and to support them in maintaining sobriety and thus a better quality of life.  The program offers primary care providers the opportunity to enhance learning, share best practices, increase the availability of skilled consultation as well as access to meeting patients’ needs for services in substance abuse, mental health, primary care, and other supportive services.
