Pulmonology/(Allergy) Elective Information

Goals and Objectives of Rotation:

  1. Understand how to take a structured pulmonary disease history, focusing on pertinent aspects of the patient and family history in common pulmonary disorders including asthma, cystic fibrosis, pneumonia, bronchiolitis, apnea, and chronic lung disease of prematurity.
  2. Understand how to perform a focused physical examination including inspection, palpation, auscultation and percussion. Recognize common pulmonary signs including chest wall symmetry, hyperpnea, hypopnea, and paradoxical breathing; respiratory rhythms such as periodic breathing; and an ability to judge respiratory effort or dyspnea. The resident will gain knowledge regarding the subjective perception of respiratory acoustic signs including wheezes, crackles, pleural rubs, stridor, and grunting.
  3. Understand how to evaluate and manage common pulmonary conditions treated in inpatient and outpatient settings including apnea, bronchiolitis, asthma, pneumonia and cystic fibrosis.
  4. Understand the indications, interpretations, costs and limitations of common laboratory and imaging studies utilized in pulmonary patient care including chest radiographs, sweat tests, and physiologic testing and monitoring.
  5. Develop a thorough and efficient approach to the care of pulmonary patients, including focusing the questions raised in a consultation and learning to address them succinctly.
  6. Support and advocate for patients while coordinating care provided by multiple services.
  7. Understand how to maintain accurate, timely, and legally appropriate medical records for pulmonary patients including:
    1. History and pertinent physical examination;
    2. Problem list/working and final diagnoses;
    3. Initial plan;
    4. Results of diagnostic studies;
    5. Discharge instructions with evidence of written instruction to the caretaker. Effectively communicate in writing and/or by phone with referring and primary physician as indicated as well as other caretakers.
  8. Learn to critically read and critique the pulmonary medical literature
