Before Your Appointment
Before you call for an appointment with the Pediatric Neuromuscular Clinic, please check with your insurance if any preauthorizations and/or referrals are needed. Then please register your child with MassGeneral Hospital for Children by calling 1-866-211-6588.
Once your child is registered, please contact our clinic coordinator Laurie Bliss at 1-617-840-0528 to schedule your child for the Pediatric Neuromuscular Clinic Pipeline or for an individual appointment.
Intake Packet
Prior to your appointment, you will receive an intake packet, including the locations of your child’s appointment, a confirmation letter, directions to MassGeneral Hospital for Children and an intake form. Please review the materials carefully and fill out the intake form before you come to the hospital. Please send your child’s previous medical records to:

Examples of Medical Records and Tests that are Helpful
- MRI or CT imaging (original copy of the images on film or CD disc, in addition to the written report of the study).
- DNA genetic testing, Serum CK (creatine kinase) blood or spinal fluid studies,
- Neuropsychological testing
- Early intervention or school based developmental testing
- Current IEP from School
- Electromyography-(EMG)
- Nerve-Conduction-Velocities (NCV)
- Electroencephalogram (EEG) reports
- EKG/ECHO
- DEXA
- Sleep Study
- Discharge summaries from any recent hospital admissions
- Reports from past neurologists or rehab doctors
- Relevant medical records from relatives with similar problems esp if any DNA testing done
Volunteer Appointment Buddies
You can request a volunteer appointment buddy to escort you to your appointment(s), carry light bags, and play or read with your children. To request a buddy call 617-840-0528 or email PedsNM@partners.org at least three business days before your appointment(s). Buddies wearing pink volunteer jackets and will meet you at the front desk of the Yawkey Building just inside the door from the valet drop-off.
What to Bring
Please note that the day of your child’s comprehensive neuromuscular evaluation can be long and arduous but our goal is to offer multi-disciplinary coordinated clinic appointments all on the same day for your family’s convenience. You might want to bring favorite toys and books, snacks, drinks, bottles, diapers, wipes, etc.
Other materials to remember to bring:
- Child’s medications
- Child’s medication history
- Day/night braces
- Shorts for physical therapy exam
- A hard copy of records
- Other entertainment for your child
