As a parent at MassGeneral Hospital for Children, you are not alone. In addition to our world-class clinical services, we have a network of support that ranges from backup childcare for siblings to financial assistance counselors, child-life specialists, a state-of-the-art patient-and-family learning center, and much more.

For families of children with medically complex healthcare needs, our Coordinated Care Clinic provides help with every aspect of a child's care, including:

  • Arrangement of visits with pediatric specialists
  • Facilitation of communication and collaboration among specialists
  • Development of comprehensive, coordinated treatment plans
  • Consultations with social workers, physical therapists, and other allied health professionals
  • Help with diagnostic dilemmas

At the Coordinated Care Clinic, your child becomes the center of a team effort. We work hard to ensure that all providers are able to collaborate effectively, facilitating communication among primary care physicians, specialists, therapists, schools, and families, to provide the continuity of care children with complex medical conditions need.

Continuity of Care and Ongoing Support

The Coordinated Care Clinic also provides continuity of care over time. For families of children with chronic conditions, we are here today, tomorrow, and next year--we're available to your family for as long as you need us.

We provide transitional outpatient care for children discharged from the neonatal and pediatric intensive care units and the inpatient wards and assistance to families dealing with a new diagnosis, new medical equipment, or new medications.

We help children transition back to home and into the community, assisting with school and education plans, transportation, and community services.

We aim to improve the quality of care given to patients and families facing a life-threatening illness by working closely with the Pediatric Palliative Care Service.

The Team

The Coordinated Care team consists of healthcare professionals with broad experience caring for medically complex children. MGHfC pediatricians Carl Seashore, MD; Marjorie Curran, MD; Kerstin Zanger, MD; Jeanne Van Cleave, MD; and Katharine Zuckerman, MD, direct medical work-up, collaborative consultation with specialists, and admission to the hospital if needed. Nurse practioner Mark Robel, NP, has over ten years of experience caring for children with special healthcare needs. Other team members include pediatric social worker Pamela Bowers, LICSW; physical therapist Anne Chastain; family advisor Sandra Clancy, and palliative care expert Patricia O'Malley, MD.
