About Us
The DSME program at the Chelsea Health Center begins with a comprehensive individual visit with a diabetes nurse educator. Participants may then proceed to a series of group education sessions or one-on-one visits facilitated by a diabetes educator. Patients are encouraged to bring a family member or friend for support.

Our Programs
Diabetes Self Management Education classes are offered monthly at the Chelsea HealthCare Center for adult patients diagnosed with Type 2 diabetes. Some classes are conducted in Spanish and American Sign Language. Please contact us for more information.
The Diabetes Self Management Education program at the Chelsea Health Center has been Recognized by the American Diabetes Association for Quality Self-Management Education.
