Learn more about Diabetes Self Management Education and Support (DSME/S) Programs at the Charlestown HealthCare Center
The DSME program at the Charlestown HealthCare Center begins with a comprehensive individual visit with a diabetes nurse educator. Participants may then proceed to a series of group education sessions or one-on-one visits facilitated by a diabetes educator. Participants are also able to attend a hands-on cooking classes taught by a trained chef where they will learn how to prepare a diabetic friendly recipe, help prepare their own lunch and receive the recipe to try at home. Patients are encouraged to bring a family member or friend for support
Diabetes Self Management Education classes are offered monthly at the Charlestown HealthCare Center for adult patients diagnosed with Type 2 Diabetes.
Diabetes Support Groups are offered monthly at the Charlestown Healthcare Center for adults diagnosed with Type 2 Diabetes. Attendees are welcome to drop in and bring a friend/family member for support.
The Diabetes Self Management Education program at the Charlestown HealthCare Center has been Recognized by the American Diabetes Association for Quality Self-Management Education.
