Mass General Senior Health Recognized as a Patient Centered Medical Home
Mass General Senior Health has been recognized as a Level 3 NCQA Patient Centered Medical Home. Medical Home Recognition is based upon a practice proving, through systems and documentation, that they have a model of care that is based around quality and safety. Quality is achieved through enhancing communication and monitoring patient outcomes. The guiding principles of a Medical Home result in a more personalized, coordinated, effective and efficient care of patients. The model advances the use of systematic processes and IT to enhance quality care of patients and promotes the principles of a Medical Home: Personal physician, physician-directed medical practice, whole person orientation, coordinated and integrated care, quality and safety and enhanced access.
NCQA Recognized medical homes clearly deomonstrate that systems and processes are in place to meet nationally recognized standards for delivering high-quality care. Mass General Senior Health received a Level 3 Recognition, the highest level possible. The lengthy application process was completed in July 2010 by Barbara Roberge, NP, PhD, Samantha Sawdon, Senior Health Practice Manager and Jeffrey Holmes, Project Specialist, Mass General Radiology Administration.
