Skilled Nursing Care and Rehabilitation Services
If you require post hospital care in a rehabilitation or skilled nursing facility a member of our practice will continue to treat you if you are discharged to one of the following facilities: Eastpointe Rehabilitation Center-Chelsea, The North End sub –acute or long term care units at the North End Nursing Home-Boston, Clark House-Westwood and Lasell Village in Newton.
Upon discharge from any care facility we request that you make a follow up appointment with your physician or our nurse practitioners. Please call us at 617-726-4600.
