Get Started If you are a new patient, you may complete the form below to request an appointment. Our outpatient access nurse will ask you more about your condition and symptoms, and match you with the best-fitting Heart Center program and physician. Make an appointment with one of our specialists

After submitting the online form, the Access Office will contact you within two business days. If you are already a patient of a Heart Center cardiologist or cardiac surgeon, please call your physician's office directly if you wish to schedule a follow up appointment.

If you are experiencing a medical emergency or a life threatening situation call 911 immediately.

After You Request an Appointment

Once the online appointment request is received, a member of our Heart Center access team will call and ask you to:

• Provide available demographic information
• Explain your diagnosis and/or complaint in more detail (reason for contacting the Heart Center)

After providing this background information, you will need to call the Mass General Patient Registration line at 866-211-6588. After registering, you will receive a patient "blue card" number. A blue card is our way of tracking patient information, including insurance providers and important contact information.

• Medical history and past physicals
• Medication list
• Stress testing results
• Holter monitor results
• Echocardiogram results
• Prior catheterization results
• Any operative reports

Arranging an Appointment With Our Specialists

Once we receive your medical background information, our Heart Center access nurse will set up an appointment with the best physician for you. Please note that depending on your diagnosis, symptoms and insurance coverage, the Heart Center may require a referral from your primary care physician. The Access Office staff will call you with the appointment details once completed.
