**** IMPORTANT PAYMENT INFORMATION ****
Because of increasing difficulties in obtaining reimbursement for our specialized testing, we can no longer accept insurance [commercial or managed care] for payment. We will provide an itemized invoice upon completion of testing for your use in obtaining reimbursement directly from your insurance plan.
We apologize for the inconvenience this causes but must establish this policy in order to insure the continued operation of this not-for-profit diagnostic laboratory.
The following options for test payment are available. Please indicate clearly
on the requisition form the selected method of payment.
* Patients may prepay directly for DNA Diagnostic testing with check or credit card (VISA or Mastercard).
* Direct billing to referring institution. Please enclose a statement to this effect on institution letterhead and indicate to whom the bills should be submitted.
* Medicare and out of state Medicaid is not accepted. Massachusetts Medicaid is only accepted if prior authorization in writing is received.
If sample is received in the lab without payment or accepted billing information, we will hold it and not proceed with testing until appropriate payment or billing information is received.
