Appointments
A family member should contact the Memory Disorders Unit Coordinator Christine M. Cunha at 617-726-1728 to discuss the patient's symptoms and to request an appointment. A family member should attend appointments with the patient to help provide accurate histories.
To request a consult from the Memory Disorders Unit, please ask your primary care physician to complete a Referral Form (PDF) (also available as a MS Word file) and fax it to the Memory Disorders Unit number on the form.
If a patient is new to MGH, please call the Massachusetts General Hospital Registration Center at 1-866-211-6588 to get a patient number and “blue card”. MGH Neurology will contact the patient directly to schedule an appointment. Office hours are Thursday and Friday from 9:00 to 4:00. Patients are seen on Monday and Tuesday afternoons.
