Referrals

Primary care physicians can call the Movement Disorders unit coordinator at 617-726-5532 to refer patients. Please complete a Referral Form (PDF) (also available as a MS Word file) indicating the appropriate subspecialty and/or Neurology coordinator and fax to the number on the form. Patients referred to Neurology will need to have an insurance referral generated prior to their visit.

Referring physicians receive a copy of the patients' evaluation as well as recommendations for diagnosis and follow-up treatments.
