Consultations and Referrals

To request a consult from the General Neurology service or from a specific Neurology subspecialty clinic, please ask your primary care physician to complete a Referral Form (PDF) (also available as a MS Word file) and fax it to the subspecialty number on the form. Appointments

Before making an appointment, please call the Massachusetts General Hospital Registration Center at 1-866-211-6588 to get a patient number and “blue card”.

Billing and Insurance

MGH Customer Service Department at 617-726-2040.
So that they may provide you with the most complete and accurate billing service, please have the following information at hand:

Your MGH Blue Card
Your current insurance information
A copy of your referral from your primary care provider (if required)
Your co-pay amount
Your outpatient visit or inpatient admit and discharge dates

Doctors and patients may register to receive online consultations with a specialist at MGH or other Partners hospital without coming to Boston. Fees for services are not covered by insurance
