Electronic Medical Records

As part of the Partners HealthCare network, NSPG doctors and hospitals are able to share secure electronic medical records and follow common standards and protocols for care. This technology enables our physicians to measure quality, determine the best course of treatment, and closely track each patient’s care. If a patient is ever hospitalized, his or her physicians can access up-to-date medical information immediately.

Medical Home

Patient-centered medical home is an integrated approach to care practiced by North Shore Physicians Group. Primary care physicians lead a team of care providers focused on the needs of each patient. Nurses, care managers, nutritionists, behavioral health specialists and pharmacists work in conjunction with the primary care physician to manage the patient’s care in a proactive manner, with a focus on wellness and prevention.

In the medical home model, a patient with diabetes will come in once a year for a physical and then every three months to have a blood test. In addition, that patient will meet with a nurse specialist between visits to discuss medications and lifestyle issues. This nurse will also be available by phone or email to answer questions. This approach helps keep patients healthier and out of the hospital, and as an added benefit, helps reduce everyone’s healthcare costs. Learn more

Shared Appointments

North Shore Physicians Group (NSPG) offers shared medical appointments for patients with a variety of diseases or conditions. Selected NSPG physicians offer 90-minute appointments, which are conducted in a group setting with a physician, multidisciplinary team and six to ten other patients.

Individual consultations and benefits of shared appointments

Shared medical appointment patients also receive an individual consultation and review of medications or tests. During the shared appointment, patients get the benefit of the group education session and of sharing the experiences of other patients. All participating patients sign a confidentiality agreement.

Current shared medical appointments include:

  • Primary care (adults)
