Patients can now view much of their medical record information online 24/7 using the Partners Patient Gateway. Go to www.patientgateway.org to enroll. You can choose "Medical Records, MGH" as your provider or select your provider/practice if they are listed.
The fastest and easiest way obtain a copy of your medical records at Mass General, is to submit a signed and dated authorization form via fax. Click here to complete the English (PDF) or Spanish (PDF) version of this form. The form should be filled out electronically then printed and signed. The form can then be faxed to 617-726-3661 to the attention of “MGH Release of Information”.
Faxing the form to us is the fastest and easiest way to obtain copies of your records, however, if you are unable to fax the completed and signed form please mail it to us at:

Medical records can include medical history, pathology, radiology, lab reports and operative reports, which will be sent by mail or secure email to the address you provide. To obtain copies of radiology images or films, please contact the Radiology Imaging Services Center directly at 617-726-1798. Please note that Mass General does not provide birth or death certificates. Please be aware that a fee may be associated with your request, and you should allow up to 30 days for processing.
If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative is required. For recently deceased patients, the request must be accompanied by documentation showing the executor / executrix of the estate. Additional authorization may also be required for the release of specifically protected or privileged information. Contact the Release of Information Unit at 617-726-2361 with any questions.
Please be advised that the Somerville location is not a patient walk in facility. For urgent medical record copy requests, patients can visit our medical records copy office conveniently located on the hospital's main campus on the 8th floor of the Founder's building. If you prefer to receive your records electronically please make certain you clearly indicate this on your request and provide a valid email address. We will send a link to you via email which will allow you to safely log into our secure delivery server and download a .PDF version of your records.
