Urgent Medical Record Requests

For urgent medical record copy requests, patients can visit the medical records copy office at
Mass General, located on the eighth floor of the Founder’s Building.

Medical records include a patient’s medical history, pathology, radiology, lab reports and operative reports of treatments and medical services. Patients at Massachusetts General Hospital can obtain copies of their medical records by submitting a form to the Mass General Medical Records Release of Information Unit.

You may receive your medical records by mail or secure email. If you request electronic records, you will receive a link via email with access to a secure delivery server.

To request your patient medical record:

  1. Download and complete a
    medical information release authorization form (PDF)
  2. Print and sign the completed form
  3. Fax the form to 617-726-3661, Attn: Mass General Release of Information

Faxing the form to us is the fastest and easiest way to obtain copies of your records. However, you may also request a copy of your records by sending a completed and signed printout of the medical information release authorization form to:

If the patient is a minor or unable to give consent, the signature of a parent, guardian or other legal representative is required.

Additional authorization may also be required for the release of specifically protected or privileged information. Certain information can take up to 30 days for processing. Contact the Release of Information Unit at 617-726-2361 with questions about specific requests.

Mass General does not provide birth or death certificates. To request medical records of a deceased patient, the request must be accompanied by authorization from the executor of the estate.

Please note that a fee may be associated with a medical record request.

Patients also have secure online access to much of their health information 24/7 using the Partners HealthCare Patient Gateway. Go to www.patientgateway.org to enroll. Choose “Medical Records, MGH” as the provider or select a specific provider or practice if they are listed.
