PROFILE
Nephrology practice includes the care of patients with impairment of renal function associated with complex diseases and disabilities. Treatments are provided in the following settings: a) seven station special care Hemodialysis Unit, b) Transplant Unit, c) Intensive Care Unit (s) (medical, surgical, neuro, pediatric, and d) in the patient's room if deemed medically necessary.

UNIT FOCUS
We support a highly active Transplant program, (renal, liver, heart and lung) as well as the critically ill patients with concurrent acute renal failure. We serve a; "backup unit" for patients on

maintenance outpatient dialysis requiring interim medical and for surgical intervention in an acute setting or admission to the Massachusetts Eye and Ear Infirmary. The level of care is highly specific and directed towards reducing morbidity and mortality. A greater degree of interdisciplinary collaboration among health care providers is required as is dependence on highly specialized knowledge and technology. We also provide plasma exchange to patients with hepatic failure pre and/or post-liver transplantation among patients having Goodpasteur"s Syndrome, Wegener's and ITP requiring hemodialysis and plasma exchange. In addition, hemodialysis or henioperfusion can be performed on an acute basis for the treatment of selected drug overdose and poisoning.

PATIENT CARE DELIVERY MODEL
The patient care delivery model is a patient-focused model, supported by primary nursing. Patient-focused care is high quality, comprehensive, accessible, supportive and personalized care. Primary nursing is practiced on the Unit. The most common nursing diagnoses are alteration in electrolytes, alteration in volume, alteration in circulation re: access, and knowledge deficit re: coping.

SCHEDULING PRACTICES
Staff schedules are driven by patient need. The scheduling process is staff driven and flexible to accommodate personal and educational needs. Once the schedule is posted, the staff manages changes. Dialysis is provided seven days per week. One RN is on call after regular hours. Dialysis patients are scheduled for hemodialysis six days per week. Two patient shifts are available in the dialysis unit; there is a 1:1 nurse/patient ratio for treatments done outside the dialysis unit.

STAFF ORIENTATION
All staff attends a one- week nursing orientation program. Unit orientation is preceptor based, individualized and determined by the Nursing Director, clinical nurse specialist and the new employee. The length of orientation is also individually determined.

EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
Staff education and development is supported by The Norman Knight Nursing Center for Clinical & Professional Development. Full and partial day programs are offered. Staff may also access outside educational opportunities. The teaching environment provides staff with a daily opportunity to discuss patient care related educational issues with members of the graduate medical education program and with other members of the care-delivery team.
