PROFILE
Ellison 14 is an integrated 26 bed inpatient hematology, oncology bone marrow transplant unit that serves a young adult to elderly patient population. Patients are frequently followed from time of diagnosis or referral through end of life. The majority of patients have a diagnosis of leukemia, non-Hodgkin's lymphoma, sarcoma, or multiple myeloma. Patients admitted to this unit require complex care focusing on multi-modality treatment that includes high-dose combination chemotherapy, radiation, biotherapy, and bone marrow/peripheral stem cell transplants comprising both autologous and related allogeneic including mixed chimerism. The goal of patient care spans a continuum that extends from a curative to supportive/palliative care focus. Complexity of care includes management and treatment of side effects and complications of treatment, administration of blood products, pain management, patient/family education and emotional/spiritual support. The most common nursing diagnoses include risk for side effects related to intensive/high dose chemotherapy, risk for injury related to severe immuno-suppression, pain, knowledge deficit related to new diagnosis and/or treatment.

UNIT FOCUS
Staff Nurses working on Ellison 14 are certified to administer IV chemotherapy and blood products and are required to complete competencies related to the care of the bone marrow transplant patient population. Ellison 14 fosters and promotes the development of expertise in oncology nursing practice. Unit specific orientation and programs have been implemented to facilitate the orientation and ongoing development of both experienced RNs and new graduate nurses. Membership in the Oncology Nursing Society and attainment of Oncology Nurse Certification (OCN) is valued and strongly encouraged. Departmental funding is available to staff applying for OCN certification.

PATIENT CARE DELIVERY MODEL

TEAM MEMBERS
A team of health care personnel consisting of physicians (i.e., attendings, fellows, residents/interns); physician assistant; registered nurses (i.e., nurse practitioners, clinical nurse specialists, staff nurses); case manager; pharmacists; social workers; dietitians; chaplains; physical, occupational, speech therapists and consult services (i.e., Palliative Care Services, Pain Service; Infection Control) work collaboratively to provide twenty-four hour coverage and support to patients and family members. Registered nurses are assisted by patient care associates.

SCHEDULING PRACTICES
Staff schedules are driven by patient need. The scheduling process is staff driven and flexible to accommodate personal and educational needs. Once the schedule is posted, the staff manages changes. A self -scheduling model is in place on Ellison 14. A Time Planning committee consisting of elected members of the Ellison 14 staff oversee the development, implementation and evaluation of scheduling guidelines and finalization of time plans. Full, part time and per diem staff is employed on the unit. Flex hours may consist of 4, 8, 12 hour shifts.

STAFF ORIENTATION
All staff attends a one- week nursing orientation program. Unit orientation is preceptor based, individualized and determined by the Nursing Director, clinical nurse specialist and the new employee. The length of orientation is also individually determined. New nursing graduates participate in the New Graduate Mentorship Program.

EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
Staff education and development is supported by The Norman Knight Nursing Center for Clinical & Professional Development. Full and partial day programs are offered. Staff may also access outside educational opportunities. The teaching environment provides staff with a daily opportunity to discuss patient care related educational issues with members of the graduate medical education program and with other members of the care-delivery team.

Clinical Nurse Specialists provide educational support to staff on the nursing units. Staff has time allocated and the necessary support to offer educational programs to other staff members. Nursing Grand Rounds programs are offered on a regular basis.

Staff RNs have the opportunity to participate as a nurse preceptor or a team leader. RNs may advance in their clinical practice and demonstrate proficiency through the Clinical Recognition Program. Several awards specific to oncology nursing practice are available to staff. Staff attendance at local, regional and national conferences is encouraged and supported. Participation in the MGH Collaborative Governance committee structure provides staff the opportunity to develop leadership skills while collaborating and working with colleagues throughout the Department of Patient Care Services. Flex time for staff enrolled in degree programs is available.
