PATIENT CARE DELIVERY MODEL
The patient care delivery model is a patient-centered
model, supported by primary nursing, providing high quality, comprehensive, accessible, supportive and personalized care.
TEAM MEMBERS
Registered nurses plan, implement and evaluate patient care in association with the multidisciplinary team consisting of physicians, surgeons, physician's assistants, nurse practitioners, respiratory and physical therapists, social workers and case management.
Patient care associates support the RN in the care of the patient. The support staff consists of operation associates and unit service associates. To inquire about positions
available on this unit, contact Christina Vasiliou at cvasiliou@partners.org.
SCHEDULING PRACTICES
The scheduling process is staff driven
and flexible to accommodate as many personal and professional needs
as possible. It is managed by a staff nurse and overseen by the
Nursing Director. Flexible scheduling includes; shift rotation, voluntary
permanent off shifts, eight and twelve hour shifts, long week, short
week opportunities, and special weekend time (includes extra differential).
STAFF ORIENTATION
All staff attends a one-week nursing orientation
program. Unit orientation is preceptor based, individualized and
determined by the Nursing Director, clinical nurse specialist and the
new employee. The length of orientation, while averaging 8 weeks in length, is also individually determined.
Instructional and clinical learning opportunities are planned to expand over a one year period to support the needs of this buy unit and strengthen clinical, organizational and critical thinking skills.
EDUCATIONAL AND DEVELOPMENT OPPORTUNITIES
Staff education and development is supported
by the clinical nurse specialist and The Norman Knight Nursing Center for Clinical & Professional Development. Full and partial
day programs are offered. Staff may also access outside educational
opportunities. The teaching environment provides staff with a daily
opportunity to discuss patient care related educational issues with
members of the graduate medical education program and with other
members of the care-delivery team.
Clinical Nurse Specialists provide educational
support to staff on the nursing units. Staff has time allocated
and the necessary support to offer educational programs to other
staff members. Nursing Grand Rounds programs are offered on a regular
basis.
Staff RNs have the opportunity to participate
as a nurse preceptor or a team leader. RN's may advance in their
clinical practice and demonstrate proficiency through the Clinical
Recognition Program. Quality improvement projects offer opportunities
for professional development. Staff retreats have offered opportunities
for staff to voice their concerns and work constructively toward
teamwork and setting standards for practice.
