Fellowship Objectives:
The goal of the hand fellowship in the Division of Plastic and Reconstructive Surgery at Massachusetts General Hospital is to provide a foundation of knowledge for the basic science and surgical practice related to normal function and pathology of the upper extremity musculoskeletal system, nervous system and microcirculation. Fellows participating in this program are expected to become proficient in the clinical diagnoses and treatment methods, including rehabilitation, for acquired and congenital conditions in the hand and upper extremity.

The objectives of the fellowship include the attainment of a comprehensive knowledge base and set of surgical skills necessary for competency in the following areas of concentration: 1) problems of the hand and wrist in the ambulatory care setting, 2) problems of the hand and wrist in the inpatient setting, 3) surgical proficiency in surgery of the hand and wrist, and microsurgery.

The following are the rotation specific objectives for the MGH Plastic Surgery Hand Fellowship.

i. Massachusetts General Hospital (Plastic Surgery Service)

a. Develop skills for the evaluation of hand emergencies.
b. Develop skills for treating hand trauma and microvascular surgery.
c. Reconstruction of arthritic and post-traumatic conditions.
d. Develop skills for resident and student education. Develop practical ethical guidelines for patient care. Develop skills to manage patient care independently.
e. Learn office procedures (i.e. coding, scheduling, etc.).

ii. Massachusetts General Hospital (Orthopedic Service)

a. Develop skills for the evaluation and treatment of radius, ulna, and elbow disorders.
b. Become proficient in diagnostic and therapeutic wrist arthroscopy.

iii. Shriners Burn Institute

a. Develop skills for the reconstruction of post-traumatic and burn conditions in the upper extremity.
b. Develop skills for treating congenital hand problems.

iv. Children's Hospital/Beth Israel Deaconess Medical Center

a. Develop skills for diagnosis and treatment of congenital and pediatric acquired upper extremity conditions.
b. Become proficient in microvascular reconstruction in the upper extremity and trunk.

A strength of the hand fellowship is the graded responsibility which the hand fellow experiences throughout the calendar year. As a board eligible plastic surgeon, the hand fellow is capable of providing surgical care for many straight-forward cases, as has been demonstrated during their plastic surgery residency. With graded responsibility the fellow matures and becomes independent throughout the calendar year in the execution of progressively more complex diagnosis and treatments, both in and out of the operating room.
