Physical Therapy Appointments

We are delighted you have chosen MGH Sports Physical Therapy as your physical therapy provider. To help you through the registration process, we have provided some useful information below.

Insurance Coverage

  • Patients with HMO plans need to contact their Primary Care Physician’s office and request an insurance referral to cover their physical therapy visits.
  • Patients with PPO plans need to contact their insurance company to verify any limitations on functional therapies (e.g., physical therapy, occupational therapy, speech therapy).

Getting Started

First, you will need a medical record number (MRN, or "Blue Card") from MGH. If you do not already have one, please call our central registration line at 1-866-211-6588 with your contact and insurance information, and they will assign one to you.

Once you have your MRN, you can register for an appointment with us in one of two ways:

  • In person, on the 4th floor of 175 Cambridge Street, Boston, where our office is located; or
  • By calling our main number, (617) 643-9999 during our regular business hours: Monday - Thursday 7 a.m. - 7 p.m., and Fridays 7 a.m. - 6 p.m.

When making your appointment, we will ask you for your:

  • MRN / Blue Card
  • Doctor's order or doctor's referral
  • Insurance card
  • Date of surgery (if applicable)

While confirming your insurance coverage we may instruct you to call your:

  • Primary Care Provider (PCP) for an insurance referral and give you our provider name and number to give them and our fax number (617) 643-0822; or
  • Insurance company to confirm your rehabilitation or functional services benefit.

Forms to fill out prior to your visit:

You can expedite the check-in process at your first appointment by downloading, printing, and completing the Health Status Questionnaire and Functional Activities Questionnaire at home, and then bringing them with you to your appointment.

At the time of your first visit we will:

  • Confirm your name, appointment time, and demographic information.
  • Ask you for your doctor’s order or referral, unless you gave it to us when you made the appointment.
  • Make a copy of your insurance card.
  • Ask you to pay your co-payment (necessary at each visit), if applicable.

If you have been seen at a facility outside the Partners network before, please bring in all radiological reports (x-ray, MRI) of your injury and operative reports. This will enable the therapist to have the most complete information in assessing your treatment plan.

At the time of your visit, please bring comfortable or sport-type clothing. If you need to change, you may use our on-site locker room facilities.
