Introducing Care Managers to a Primary Care Practice 101

The John D. Stoeckle Center and PCHI proudly announce this 3.5 hour workshop featuring expert faculty from across the PHS network.  This conference will focus on our methods to identify high risk patients and how to successfully introduce care managers into the primary care team.  Team-based care and practice redesign are critical components of the Patient-Centered Medical Home, a major focus for Partners Healthcare System in 2012.

Lead faculty from the MGH, BWH, NSMC Care Management Programs will discuss how to identify high risk patients and the successes and failures they've experienced when caring for these patients. The workshop will provide an in-depth discussion of the role and function of a care manager and the "day in the life" of care managers from our network. Dyads of physicians and care managers will lead breakout participants through an actual case and review how they work together as a team to provide the best care for the patient.

Intended audience

Care Managers and physician, nursing, administrative leadership at the RSO and practice level.
