Partners Medical Home LEAN Collaborative: The Next Step in Practice Re-Design

The Partners Medical Home LEAN collaborative is a formal follow up program consisting of four sessions over three months for physicians and members of their team who have previously participated in Virginia Mason Workshops to redesign their practices.

Scope:

  • Network Lean Experts: The program is facilitated by LEAN experts Mary Cramer, MBA, CPHQ, MGH, Process Improvement Program Director and Director, Ambulatory Management and Performance Consulting Team and Dorothy Goulart, MS, RN, BWH, Director for Performance Improvement. 
  • Coaching: Medical Home Consultants Colleen Blanchette and Jackie Quinn will support participants as they launch practice improvement projects.
  • Practice Improvement and Best Practice sharing: Participants will improve practice processes through an in-depth project and learn from the experience of other teams as they exchange best practices on a defined set of initiatives.
  • Sponsorship: Each team will have an Executive sponsor (e.g. RSO or practice executive or clinical leadership) that will act as the champion for the team and provide focus and resources to ensure projects are aligned with relevant strategic priorities.

Target Participants:
Participants from the Virginia Mason workshops (e.g. physician, MA (Flow Manager) and Practice Manager)
