The Mass General Tobacco Treatment Service is a multi-disciplinary team of health professionals who specialize in helping smokers become nonsmokers. We offer low-cost, state-of-the-art tobacco treatment services to all Mass patients, employees, and other members of the community.

If you are a Mass General physician or other clinician, you can refer a smoker to us by using a form available on the Mass General Primary Care Office InSite (PCOI) web page. To access this form:
- Go to this website: http://oi.mgh.harvard.edu
- Click "Useful Forms" and select "Smoking Counseling/ Referral Form" under the "Referrals" link.
- If you click on the icon, you will go to a printable version of the form that can be sent via fax or mail; if you click on the text, you will go to an electronic version that can be sent via e-mail. You will not be able to access this form outside of the Partners network.
