If you are an MGH physician or other clinician, you can refer a smoker to us by using a special form available on the MGH Primary Care Office InSite web page. To access this form, click here, then go to "Useful Forms" and select "Smoking Counseling/ Referral Form." If you click on the icon, you will go to a printable version of the form that can be sent via fax or mail; if you click on the text, you will go to an electronic version that can be sent via e-mail. You will not be able to access this form outside of MGH.
