Diagnosis or Reason for the Visit
Insurance Information (if you have an HMO or some other managed care plan, please provide the Secretary with a referral number and authorization from your primary care physician).
For second opinions or other consultations, please provide the Urologist with any:X-rays or other radiology films (CAT scans, bone scans, etc.) and/or reports
Copies of referring physician’s office notes
Pathology reports
Pathology slides (if available)
Prior to your appointment, it will be necessary to get an MGH Registration Card ("Blue Card").
