Building Healthier Communities • CCHI at 25 Years
To honor the Center for Community Health Improvement’s 25th anniversary, we sat down with colleagues and staff whose experiences embody CCHI’s key strategies: empowering youth and promoting educational attainment, improving access to care and engaging communities to build a culture of health.
The social determinants of health—factors such as housing, education and employment—account for 80% of our health status.
Focusing on these social determinants, we seek to eliminate health inequities based on socioeconomic status, race and ethnicity. We leverage prevention, early intervention and treatment approaches that are measurable and have proven impact.
CCHI supports a range of programs that reduce social and economic barriers for vulnerable patients, including community health workers, navigators and outreach programs.
Empowering Youth and Promoting Educational Attainment
Mass General Youth Programs provide more than 1,000 underrepresented students from grades 3 and up with opportunities in health and science with the goal of sparking college aspirations and beyond.
CCHI provides leadership and backbone support to four multisector coalitions in Revere, Charlestown, Chelsea and East Boston to address urgent health challenges like substance use, obesity and trauma.
Tackling issues like substance use, hunger, obesity or safety can seem overwhelming, but at CCHI, we believe in prevention and collaboration. With our community partners, we are working to make entire communities, not just individuals, healthier.

89% of patients working with community health workers achieved health goals
improving medication adherence, connecting to community-based resources and increasing healthy behaviors like physical activity and healthy eating.
We work with 2,100+ youth in our core educational programs,
through our summer jobs program and with community youth groups.
Working with 200+ community partners, we serve more than 150,000 residents
in Charlestown, Chelsea & Revere.
