Request for pathology material for Patient Care and Treatment (Clinical Necessity) requires the following information:


• Patient’s Full Name and Date of Birth as it appears in the Mass General record
• Mass General Medical Record Number
• Patient’s Mass General provider’s name
• Date of Service and/or type of surgery – case # if available
• Name, Address (mailing and/or e-mail), telephone number of physician and institution/testing facility requesting slides
• Full name and address of person and institution/testing facility receiving the slides

Tissue Information:


• Material being requested
• Purpose of the material being requested
• Tissue Type Specifics, i.e. original slides, recut stained slides, recut unstained slides, scrolls, punch cores*
• Detailed description of preparation/procedure for the tissue

*Slides/punch cores from archival blocks may be provided, if the specific block has sufficient tissue and fulfilling the request will not exhaust diagnostic tissue within the archival block.  A Mass General pathologist must determine availability of suitable tissue, and ensure that archival tissues are not exhausted. Exceptions occur in cases where clinical diagnosis is necessary and this tissue is the only option for patient care results. This always requires pathologist approval and documentation in the LIS (Laboratory Information System).

Request for material for Research purposes requires the following information:


• Collaboration with an Mass General Pathologist
• Patient’s Full Name and Date of Birth as it appears in the Mass General record
• Mass General Medical Record Number
• Patient’s Mass General provider’s name
• Date of Service and/or type of surgery – case # if available
• Name, Address (mailing and/or e-mail), Telephone number of physician and institution/testing facility requesting slides
• Full name and address of person and institution/testing facility receiving the slides
• Copy of IRB - expiration and renewal date required
• Signed Patient Consent to participate

Tissue Information:


• Material being requested
• Purpose of the material being requested
• Tissue Type Specifics, i.e. original slides, recut stained slides, recut unstained slides, scrolls, punch cores*
• Detailed description of preparation/procedure for the tissue

Request for Pathology Material for Medical-Legal purposes:


Medical-Legal requests are processed by the Slide File Room. The required documentation is a written request which includes the patient’s Full Name and Date of Birth, signed Patient Consent (except in the case of a Court Order), and a detailed description of what is being requested.  

Expectation for Return of Material:


Mass General Pathology Service expects return of all ORIGINAL pathology materials.  The following due dates can be extended by contacting the Slide File Room:

• For Patient Care and Research: within 30 days of receipt
• For Medical-Legal purposes: within 6 months of receipt.
 
Note: There are special circumstances where recuts slides may be requested back to Mass General.  If so, this will be noted during processing and when the material is sent.
