THE CLINICAL RESEARCH CENTER (Located on White 13) - MEDICAL RECORD STORAGE FOR STUDY SUBJECTS
The Clinical Research Center located on White 13 study subjects’ medical records will be purged either to Medical Records or to Retrievex. This designation must match what is indicated on the consent form.
Medical Records are subject to be purged after 3 months of the subject completing the study.
Please remember to collect any information that you may need out of the subject's medical record in real time.
Please note that If the medical record is being purged to MEDICAL RECORDS, it will be incorporated into their Massachusetts General Hospital medical record, including the Longitudinal Medical Record–LMR.
If the medical record is being purged to RETRIEVEX / FILELINE/ACCESS (Outside Company) the medical record will NOT be incorporated with the Mass General hospital medical record.
Records that are sent to Retrievex/ FileLine/ACCESS should be only on studies that are of a sensitive nature, such as HIV, psychiatry and genetics and must have been agreed to at the start of the study.
(NOTE: Subjects' Medical Records will be purged by Retrievex / FileLine/ACCESS within 20 years of the specified purge date when the record was put into storage)
HOW TO RETRIEVE A CRC MEDICAL RECORD FROM RETRIEVEX
If the Medical Record has been purged to Retrievex / FileLine/ACCESS and you need to retrieve the subject’s medical record, please email Faith Fortune at firstname.lastname@example.org the following subject information:
1) Subject's First and Last Name
2) Medical Record Number
3) CRC SPID Number
