Will I still have my doctor if I join the Integrated Care Management Program?
Your primary care physician (PCP) will continue to manage your health care. The relationship you have with your doctor and other health care providers at Massachusetts General Hospital will not change.

Does Integrated Care Management Program cost anything?
No, there is no cost to the patient or their family for the Integrated Care Management Program (iCMP).

Where is the iCMP office located?
Our team can be found at your Mass General primary care physician’s office. Our administrative office is at 125 Nashua Street in Boston.

Does participation in iCMP affect my insurance?
The Integrated Care Management Program is one of many programs available through the primary care practices at Mass General. Many of these programs are sponsored through Mass General Population Health Management at no cost to the patient or their insurance.

I received a letter for the Integrated Care Management Program. Will someone call me?
Yes, a member of the Integrated Care Management Program will call you within 2 weeks. 

My PCP is not part of Mass General Brigham Healthcare. Can I still join the program?
The Integrated Care Management Program at Mass General only works with patients who have a primary care physician at Mass General. If you have a primary care physician at another Mass General Brigham organization, you may be able to participate through their office. We would recommend you speak with your primary care physician about this.

Can I refer myself into the program?
If you feel you would benefit from the Integrated Care Management Program, you can speak with your primary care physician.
