About the Program
The Diabetes Collaborative Care for Young Adults (DCCYA) program at the Massachusetts General Hospital Diabetes Clinical Center is designed specifically for young adults, ages 18 to 30, with Type 1 or Type 2 diabetes. This stage of life often brings unique challenges, like managing diabetes alongside school, work, moving away from home, navigating finances or insurance, and evolving relationships.
Our program connects you with a multidisciplinary team, including physicians, nurse practitioners, nurses, and a clinical social worker with expertise in behavioral health and diabetes. Together, we focus not just on managing your diabetes, but also on the education, mental health, and social factors that influence your care.
Services we offer
- Comprehensive diabetes management
- Education and support for diabetes-related technologies, including continuous glucose monitors, insulin pumps, and smart insulin pens
- Social work support and diabetes-related mental health care
- Personalized diabetes education
- Nutrition counseling
- Exercise and physical activity guidance
- On-site laboratory services
- The option for telehealth visits
- Continued care with the Mass General Diabetes Clinical Center team after age 30
How to become a patient
- Register as a patient: Call Mass General Brigham Patient Registration at 866-489-4056 to get started.
- Get a referral (if needed): You may need a referral from your primary care provider or former endocrinologist. Referrals can be faxed to 617-724-8534.
- Schedule your first visit: Once you’re registered, call the Mass General Diabetes Clinical Center at 617-726-8722 to schedule a new patient appointment. Please mention that you’re specifically interested in the DCCYA program. Your request may be reviewed by the program director before the visit is scheduled.
