The Massachusetts General Hospital /MGHfC Autism Care Questionnaire (ACQ) is designed to improve the experiences of patients with autism spectrum disorder when receiving medical services.

The Autism Care Questionnaire (ACQ) is a brief survey completed by a patient or the patient caregiver prior to a planned or unplanned medical visit.  The ACQ addresses the non-medical needs (communication methods, environmental adaptations, sensory preferences) of the patient with autism in acute care settings. Having a plan in place will help the administrative staff and medical team handle the unique needs of the individual. The ACQ is useful for pediatric and adult patients.

Once completed, the ACQ should be printed and shared with healthcare providers.
