Our team is a blend of seasoned clinical professionals who are licensed in nursing or social work and certified in geriatric care management. They are all members of the Aging Life Care Association. Our staff has more than ten years of combined experience working in complex health care systems and assisting elders and families with challenges in finding the senior health resources they need.

Victoria L. Feenstra, RN, BSN, CM

Victoria L. FeenstraNurse Care Manager

Victoria has been a longstanding employee at Massachusetts General Hospital, starting her career as a burn nurse in 2003. Victoria transitioned to the Pediatric Adolescent unit 2 years later and remained on that unit until her recent change from floor nursing to case management. Victoria has extensive experience in the care of pediatrics, adults, and geriatrics in the hospital, as well as in the community and home settings.

From an early age, Victoria spent time in the nursing home where her mother worked as an activity director. Victoria had a special bond and fondness for this population and has remained active in a variety of ways during her years as a pediatric nurse.

She gained experience in a supervisory role while employed part time for a private home care company in 1995. Victoria remained a strong advocate for her clients and was diligent in weekly visits, which provided her with the peace and assurance that only the best of care was given. She not only worked closely with her clients but also the families, care givers, and assisted living staff members.

In 2022, Victoria joined the Thomas Upham House, an assisted living facility in Medfield, as a nurse, and was able to fulfill her passion of helping the elderly on a per diem basis, all while also working full time at MGH.

Victoria is excited to be able to help assist and support her clients here at MGH PrimeCare, along with their family members and other support people. “My goal is to connect with my clients and their families in a way that they can rely on me, feel comfortable with me as one of their support people, and enjoy their lives knowing they have me as their advocate and friend.”

Kayla Hazen, RN, BSN, CCM

Kayla is a Nurse Care Manager with Mass General PrimeCare since 2019. Her background is in psychiatric nursing and private duty homecare. She has experience working in the community, residential and emergency psychiatric settings. Her clinical interests include behavioral health, dementia, palliative care, hospice and transcultural nursing. She enjoys making connections with her clients and supporting them along their journey of life. She has been a member of the Aging Life Care Association since 2020. She became an Advanced Professional Member in 2023. She received her accelerated Bachelor of Science in Nursing from Massachusetts College of Pharmacy and Health Sciences in 2011.

Jamie Yurasits, RN, BSN, CCM

Jamie is a Nurse Care Manager with Mass General PrimeCare since 2020. Her experience comes from providing care in Skilled Nursing Facilities starting as a floor nurse, unit supervisor and Care Manager. Jamie has also worked in private home care as a Nurse Care Manager where she provided care coordination and oversight to staff RNs and CNAs caring for her clients. These roles provided her with extensive insight into the challenge’s individuals face as they encounter health and/or aging issues. This foundation has been beneficial in establishing her style and approach as a care manager. It has given her the tools to support her client’s quality of life goals. Jamie’s interests include Palliative/Hospice Care, geriatrics, and dementia care. She became a Certified Care Manager (CCM) in 2018 and became a member of Aging Life Care Association in 2020. In 2023 she was promoted to the level of Advanced Professional. She graduated from University of Massachusetts Boston with a Bachelor of Science in Nursing in 2005.

Barbara Maxey, RN, BSN, CCM

Barbara joined Mass General PrimeCare in 2004. She served as Clinical Director from 2010 to 2019.  She has spent her entire professional career at Mass General and prior to joining PrimeCare was a staff nurse, head nurse, continuing care nurse and in-patient care manager. Her clinical interests include geriatrics, Alzheimer's disease and other types of dementia along with oncology and hospice care. Barbara received her nursing degree from Catholic University in Washington, D.C.  She retired in 2019 from Mass General PrimeCare. Barbara occasionally works on a per diem basis with our clients.

Janet Sobel-Medow, RN, BSN, CCM

Janet is the Clinical Director at Mass General PrimeCare, and has been a Nurse Care Manager with this practice since 2008. Since 1992, Janet solely has worked with older adults and their families in social service settings and in her own private practice. In 2001 she became an Advanced Professional Member of Aging Life Care Association. Her clinical interests include long-distance caregiving, depression and isolation in older adults, dementia, palliative care and hospice, medical care coordination and advocacy. Janet received her Nursing Degree from the University of Michigan-Ann Arbor graduating Magna Cum Laude in 1982.
