Our team is a blend of seasoned clinical professionals who are licensed in nursing or social work and certified in geriatric care management. They are all members of the National Association of Professional Geriatric Care Managers. Our staff has more than ten years of combined experience working in complex health care systems and assisting elders and families with challenges in finding the senior health resources they need.

Barbara Maxey, RN, BSN, CCM 

Nurse Care Manager, Clinical Director

Barbara is the clinical director at Mass General PrimeCare and has been a case manager since 2004. She has spent her entire professional career at Mass General and prior to joining PrimeCare was a staff nurse, head nurse, continuing care nurse and in-patient care manager. Barbara is a member of the National Association of Professional Geriatric Care Managers (NAPGCM) and in 2009 became a Certified Care Manager (CCM). Her clinical interests include geriatrics, Alzheimer's disease and other types of dementia along with oncology and hospice care. Barbara received her nursing degree from Catholic University in Washington, D.C.

Rose Anne Dioguardi, MSW, LICSW

Clinical Social Worker, Care Manager

Rose Anne is a licensed independent clinical social worker with experience in long-term care, sub-acute rehabilitation, and geriatric psychiatry/mental health. She is currently working toward a Mental Health in Aging Certificate from Boston University and her Care Management Certification. Her clinical interests include geriatrics, dementia care, and other forms of mental illness. Rose Anne obtained her Bachelor of Arts degree from Ithaca College and her Masters in Social Work from SUNY Albany.

Janet Sobel-Medow, RN, BSN, CCM

Nurse Care Manager

Janet joined Mass General PrimeCare in 2008. Previously, she had her own private geriatric care management practice and more than 15 years as a care manager in a variety of settings. Janet is a member of the National Association of Professional Geriatric Care Managers (NAPGCM) and in 2009, became a Certified Care Manager (CCM). Her clinical interests include long-distance caregiving, depression and isolation in older adults, dementia, palliative care and hospice. Janet received her nursing degree from the University of Michigan-Ann Arbor.

Kayla Hazen, RN, BSN

Kayla has been a Registered Nurse since graduating from the accelerated Bachelor of Science in Nursing Program from Massachusetts College of Pharmacy and Health Sciences University in 2011. She has experience providing compassionate care in a variety of settings, including skilled homecare, group adult foster care, and residential and inpatient psychiatric nursing. Clinical interests include dementia, hospice, transitions of care, and psychiatric and transcultural nursing.
