While families are often very familiar with transitions for their child as they grow and progress, transitioning from pediatric to adult care can seem overwhelming and can often take families by surprise. This may be even more true for youth and young adults with special health care needs who use the healthcare system more often.

In this presentation from February 2, 2022, Evangeline Galvez, MD, an attending physician in Internal Medicine at the MGH Revere HealthCare Center and Pediatric Primary Care at MGfC, presents evidence-driven elements and resources to help plan a successful individualized transition to adult care. She discusses the differences between pediatric and adult care, suggested timelines to transition and what to consider when children transition to adult care. She also shares resources to help families and their children plan for transitions.
