A Team Approach to Care
We provide health care as a team, with a focus on the individual patient. This model is often called a “patient-centered medical home.”
Each dedicated health care team includes:
- A physician and/or resident
- Medical assistant
- Administrative staff
Together, the patient and health care team will create a health care plan that is personalized and coordinated to help them stay healthy and to provide care when they need it, often with shorter wait times. The team can help patients better understand their health care needs, maintain an accurate medication list or connect them with specialists if necessary. Patients will be encouraged to play an active part in their health care by asking questions, communicating with their care team and collaborating to reach healthy goals.
Our purpose is to improve the health and wellbeing of our patients beyond their ailments. We strive to deliver high quality care and customer service, through communication and collaboration with patients, caregivers, and other members of the patient's healthcare team.
To teach and help patients understand that the new innovations, ideas, and technology that we implement are tools to help us improve communication and provide high quality care and not meant to replace human interaction between a provider and a patient.
With our team, our patients, and the primary care services we provide, we hold these values to heart:
- Customer service with honesty, trust, and communication
- Respect in diversity, gender/equality, and confidentiality
- High quality care including being efficient and using today's innovations
- Being understanding, having empathy/compassion, and providing a safe comfortable environment
